（一）关于依法缴纳社会保障资金的证明材料的说明

（1）本项目采购文件中所称“社保经办机构”是指：《社会保险经办条例》（中华人民共和国国务院令第765号）第六十条中“社会保险经办机构”，即人力资源社会保障行政部门所属的经办基本养老保险、工伤保险、失业保险等社会保险的机构和医疗保障行政部门所属的经办基本医疗保险、生育保险等社会保险的机构。

（2）提供项目开标前半年内任意连续3个月（不含投标（响应）当月）的《缴纳社会保障资金的证明材料清单》或提供依法缴纳社会保险资金证明材料。

（二）根据庆财采【2024】17号文件要求，本项目在法定公示内容基础上将依据参与供应商自愿原则，在中标结果公告中公开投标（响应）文件中的投标承诺书、特定资格证明文件、中小企业声明函/监狱企业/残疾人福利性单位声明函、投标人业绩情况表、报价明细表、开标记录表。如同意请在投标响应文件中提供承诺函（格式自拟，位置不限，内容表明“同意”即可），不同意可不提供。
